SurvivorPlan was mentioned recently in the Journal of Cancer Survivorship’s special issue titled “The History of Cancer Survivorship Centers in the US: Progress, Challenges, and Opportunities” [February 2024, Volume 18 Issue 1]
This special issue features eight cancer centers that are pioneers and leaders in cancer survivorship care delivery:
- the University of Minnesota Masonic Cancer Center
- the University of California/Los Angeles Jonsson Comprehensive Cancer Center
- the Northwestern Medicine Robert H. Lurie Comprehensive Cancer Center
- the St Jude Children’s Research Hospital
- the University of Pennsylvania Abramson Cancer Center
- the Dana-Farber Cancer Institute
- the Stanford Cancer Institute
- and the Roswell Park Comprehensive Cancer Center
The Roswell Park article features SurvivorPlan as a fully integrated part of their clinical care delivery framework, where it is used to foster communication with the patient and their primary care provider (PCP). Each new survivor receives a comprehensive survivorship care plan that includes a cancer treatment summary, potential late effects and long-term side effects of treatment, signs of recurrence, health promotion, and prevention recommendations. The survivorship care plan is reviewed with the patient at the end of the visit, and an abbreviated version of the survivorship care plan is sent to the PCP to increase awareness of the potential late effects.
SurvivorPlan has been an integral part of Roswell Park Comprehensive Cancer Center’s survivorship program since 2019, when their solution was chosen in a competitive process with distinct requirements set forth by a multi-disciplinary committee. For the last 5 years, SurvivorPlan has been a core component of the Roswell Park Cancer Institute’s survivorship program by helping to improve clinician efficiency and provide more comprehensive support for the complex needs of the program’s survivor patient population.
